Pilot Program: Medically Tailored Home-Delivered Meals for Hospital Patients

This pilot program aims to assess the impact of providing specially prepared home-delivered meals to individuals recently discharged from hospitals with specific health conditions. If successful, this could mean that in the future, people with health issues might receive nutritional support at home, potentially improving their well-being and reducing the need for re-hospitalizations.
Key points
Hospitals will be able to provide specially prepared home-delivered meals to patients with specific diseases who were recently hospitalized.
The program will run for 3 years and involve at least 20 hospitals in at least 10 different states to evaluate the meals' impact on health and care costs.
Qualified individuals will receive at least two meals per day for a minimum of 12 weeks, along with dietitian support.
