Expanding Home Palliative Care Access for Seriously Ill Medicare Beneficiaries.

This Act establishes a 5-year Medicare pilot program to test a community-based palliative care model for individuals with serious illnesses. The goal is to improve patient outcomes, enhance the care experience, and reduce unnecessary emergency room visits and hospitalizations. Crucially, this model allows patients to receive palliative care alongside curative treatments.
Key points
Provides access to comprehensive, coordinated palliative care 24/7, furnished in the patient's home or community setting, including telehealth services.
Care includes pain and symptom management, mental health services, family/caregiver support, and advance care planning.
The program targets Medicare beneficiaries with serious illnesses (e.g., cancer, dementia, heart disease) and does not exclude those who previously used hospice benefits.
