Medicare Pilot Program: Integrated Care for Mental and Physical Health Comorbidities.

This Act establishes a demonstration program under Medicare, operating from October 2024 to September 2029, to test innovations in treating individuals with co-occurring mental and physical health conditions. The goal is to improve care quality and address social determinants of health (such as food insecurity or lack of social support) impacting patients. The program aims to assist in identifying payment reforms and improving health outcomes for Medicare and Medicaid eligible individuals, as well as the uninsured.
Key points
Implementation of care innovations, including interdisciplinary teams addressing mental and physical health, and emergency department interventions.
Supporting hospitals in implementing innovations that address social determinants of health, such as nutritional counseling, medical-legal assistance, or financial support.
The program covers individuals with serious mental illness co-occurring with conditions such as chronic diseases, high-risk pregnancy, disability, or frail elderly status.
Testing annualized payment arrangements (e.g., annual lump sums) which may include financial risk for hospitals, to support the implementation of innovation plans.
