Coordinating Health and Social Services: Grants to Improve Community Well-being
This law establishes a new Interagency Council to better coordinate health and social services and assist state and local governments in improving program outcomes and cost-effectiveness. It also authorizes up to 25 grants for states, local governments, and tribes to develop social determinants accelerator plans aimed at improving the well-being of individuals in communities with unmet needs.
